Alternate Travel Form
|
| For any band trip in which a student desires
to travel to or from an event NOT being driven by either the
school-provided transportation (such as buses, etc.) or by their own parents or
legal guardians, a BISD Alternate Travel Form must be filled out,
NOTARIZED, and submitted in advance, and approved by Mr. Koch. Students are not allowed to drive themselves to or from
an event, but must be driven by an adult driver. |

completed and submitted in order to receive a band uniform. You
may obtain these forms from the band hall, or you can download them via

Forms are saved in Adobe Acrobat format, which will
require you to have Adobe Acrobat Reader on your computer to read and/or
